How to Prepare for a Facility Notarization


To ensure a smooth and successful visit, please review these key requirements before scheduling:


The Signer Must Be Alert & Willing:

By law, the signer must be alert, coherent and understand the nature of the documents they are signing. If the patient is heavily sedated or unable to communicate their wishes, we are legally required to pause the appointment.


Valid Government-Issued Photo ID: 

The signer must present a current government issued ID (such as a Driver's License, State ID, or passport). If the patient's ID is expired or they do not have one, please contact us for alternative state-approved identification methods.


Pre-Fill Your Documents:

Please ensure all forms are completely filled out before our arrival, leaving only the signature and notarial sections blank. Please note: We are not Attorney's and cannot draft legal documents for you. 


Check for Witnesses:

Some documents require additional witnesses. Be sure to check your document's guidelines. May healthcare facilities do not allow their nursing staff to act as witnesses, so please arrange to have family members or friends available if needed.
